Good Evening, This is DJ Preacher from Chicago and One of the finalist for this years prayze factor awards event. I am excited and grateful to be a part of this grand event. I was reaching out to check and see if there is anything additional forms I had to complete prior to showing up and checking in this weekend? I have bought my ticket for Saturday night. I will be getting in on Friday, September 15. I have also noticed a Friday Night Pre-Show. Is this specifically for a specific group of artist/media group?

Hi. To my knowledge we did have to fill out an intake form. That us found on the website. They also should have emailed you this information as well.
